      Needle Stick Accident Policy & Procedure

      The purpose of this document is to establish policy and procedure to be followed in the event that one of our students has a needle stick accident. This form is exactly 2 pages - both pages must be filled out. It’s the student and instructor’s responsibility to fill out the form.
