With your permission, the information gained from this completed questionnaire will be discussed with the Administrative Headquarters for Zollege. The purpose of collecting this information is to learn more about the pain points or issues prompting your decision, if any. We would prefer to discuss these with you to determine if we can attempt to resolve any issues to the mutual satisfaction of all parties. An exit interview will be scheduled and conducted afterwards to address any issues and to go over the feedback form, if requested.
