  • In the last 48 hours, have you had a fever (100.0 +)?   
  • Have you experienced any of the following symptoms in the last 48 hours:
    • Coughing,
    • Sore Throat,
    • Difficulty Breathing,
    • Muscle Aches and/or,
    • Stomach Pain
