# Texas Radiology

## These forms are designated for the Texas Radiology Submission only. Each state may have there own rules and regulations unique to your state. If you have questions please reach out to your Campus Manager for questions.

Texas Radiology forms need to be sent to your Campus Manager after the start of each starting class. You are responsible for having your class fill out the following three forms:

- [EVALUATION OF PROTECTIVE DEVICES](http://www.vandelayinstructor.com/uploads/4/2/2/5/4225068/evaluation_of_protective_devices.pdf)
- [INSTRUCTOR POLICY & PROCEDURE GUIDE](http://www.vandelayinstructor.com/uploads/4/2/2/5/4225068/updated_715_instructor_signature_-_policy___procedures_-_radiology.docx_-_google_docs.pdf)
