Purpose
Defines the process for managing radiology permit and certification requirements for dental students in Indiana to ensure compliance with state regulations.
Scope
Applies to Academic Advisors, Instructors, and administrative staff responsible for ensuring students complete radiology permit and certification requirements.
Trigger
- Students begin dental program (Week 1)
Process Overview
1. Students Complete Permit Application
- Students fill out the Application for Provisional or Student Permit during Week 1
2. Instructor Collects and Submits Applications
- Instructor collects completed applications
- Scan documents
- Email scanned copies to coordinator for processing
3. Mail Applications to State
- Mail hard copies to Indiana Department of Health
- Division of Radiology and Weights & Measures
- 2 North Meridian Street, 4 Selig, Indianapolis, IN 46204
4. Prepare Proficiency Certification Applications
- Coordinator completes certification application for each student
5. Submit Certification Applications
- Mail completed certification applications to Indiana Department of Health
