Purpose

Defines the process for students to obtain a Dental Assistant Permit and complete radiology certification requirements.

Scope

Applies to Academic Advisors, Instructors, and students completing radiology certification requirements.

Trigger

  • Students begin radiology certification process during course enrollment

Process Overview

1. Permit Application Completion

  • Distribute Dental Assistant Permit Application during first class
  • Ensure application is completed accurately
  • Application must be notarized before signing
  • Attach full-page color copy of valid ID
  • Include required application fee ($50)

2. Application Submission

  • Mail completed application to Oklahoma State Board of Dentistry
  • Ensure all required documents and payment are included

3. Permit Number Submission

  • Students send permit number to Academic Advisor
  • Advisor verifies receipt before proceeding

4. Exam and Study Enrollment

  • Academic Advisor enrolls student in SeminarWeb
  • Assign radiology study materials and exam package
  • Students receive access instructions via email

5. Final Coordination

  • Collect all student permit numbers by Week 10
  • Submit student list for invoicing
  • Process payment for exam packages
  • Guide students through course access and exam preparation

Key Rules

  • Permit number is required before exam enrollment
  • All students must be tracked and verified
