What is the Oklahoma State Licensing Process?

This article guides you through the steps of your state licensing if it applies to your state specifically!

State Testing: Radiation Safety Course administered through the Oklahoma Dental Foundation

  • This will be a 7-hour online course
  • Once the online course is complete, students will sign up to take the proctored online exam at an approved location. (Click here for approved locations)

State Licensing/Certification: Certification

  • What: Radiology Permit & Radiation Safety Course-upon completing the online course, the student will sign up to take the proctored exam at an approved location
  • When: Upon completion of the program or within 30 days of employment
  • What does the processing look like & when do they get/hear back: 
    • Your permit applications will be filled out after Class 1. You will need to send them off ASAP so that you can be registered for your Radiology Safety Class. The fee is $50. You need to be sure to send off a color copy of your driver’s license with it. (The permit application can be found here.)
      • The permit application will be mailed to the Oklahoma State Board of Dentistry- 2920 N Lincoln Blvd., Ste. B OKC, OK 73105.
      • The application will take 1-2 weeks to be processed and for the permit to be mailed to the student
    • Once you receive your permit number from the state, please make sure to send it to your Campus Manager. This is a vital piece of information needed to register you for your Radiology Safety Course. If you do not have your permit number sent in by Class 4 you will run the risk of having to pay for the course yourself. 
    • After you are registered for the Radiology Safety Course, you will receive an email from Seminar Web. It will be coming from the email address support@seminarweb.com. Please make sure to keep an eye on your email so you can register as soon as you receive it. Students will then register to take the 7-hour online Radiation Safety Course.
    • Students will also have to take 200 radiographs (only 5 of which can be panoramic) under the supervision of a dentist within one (1) year of taking the course.
    • Once you register and start the course, you are able to stop and start as you need. Be aware that in order for you to receive your attestation sheet from the Oklahoma Dental Board to take to your externship office, you will need to have completed all of the steps in the course. 
    • After you have done all of the required steps online, you will need to find a testing site near you to take the proctored exam portion of the course. DO NOT take the test online. It has to be at a proctored site! The fees for this exam may vary based on location (click here for the registration site).
    • When you complete and pass this exam, they will notify the Oklahoma Dental Board and you will be sent your attestation form. This has to be signed by the doctor at your externship office once you have completed your 40 hours. 
    • When you have this form signed you will need to send it and $10 (check or money order) to the board to have radiology added to your permit. 

 

Any other info: After the student has applied for and completed the online course, they can submit proof of completion of the course to be reimbursed for the $125 fee associated with it. (For the reimbursement form, click here.)        

 

Additional Information: Some students will begin applying for their permits prior to gaining employment. The 200 radiographs can be completed during an externship and signed off on by the doctor.
