# What are students required to submit to be admitted into class?

## A general overview of the required admissions documents

- **A valid government-issued ID** (driver’s license, passport, ID card, etc.)
- **High school diploma, GED, proof of US high school equivalency** 
    - North Carolina requires a full transcript to be submitted 
          - A high school diploma is not accepted in the state of North Carolina.
    - If a student has a foreign diploma, we must receive an evaluation of the foreign diploma that is, at minimum, equivalent of a US High School Diploma. 
          - You can get evaluations of your diploma/transcript through [Spantran](https://spantran.com/web/services/evaluations).
    - In some states, we may accept a Wonderlic assessment in lieu of the diploma requirement.

- **Current Immunization** R**ecord** with the following immunizations: 
    - Hepatitis B series, Varicella (chickenpox), MMR, and TDaP.
    - Medical students require the above items PLUS a flu shot, and a negative PPD skin test for TB (both performed within the last 12 months.)
    - If a TB test is positive, they will need to have a chest X-Ray and a blood test to confirm the skin test was incorrect and that the disease is not actively present. We cannot accept a student that has an active TB infection.
